The information provided on this site is for informational purposes only and is not intended as a substitute for advice from your physician or other healthcare professional, nor for any information contained on or in any product label or packaging. You should not use the information on this site for diagnosis or treatment of any health condition or for prescription of any medication or treatment. Always consult your physician or qualified healthcare professional before taking any medication or nutritional, herbal, or homeopathic supplement, before starting any diet or exercise program, or before adopting any treatment for a health condition.

TailoredHealth is a wellness technology company and not a medical provider, laboratory, or pharmacy. All lab testing, genetic analysis, and supplement formulation services are provided by independent third parties, which set their own pricing. While TailoredHealth facilitates access to these services for your convenience, we do not endorse or recommend specific providers, and you are free to choose any healthcare professional or laboratory you prefer.

To use the Services you will create an account, either by verifying your email address with a one-time code or by signing in with a third-party provider such as Google. You are responsible for keeping your email address and sign-in method secure, and for all activity that occurs under your account. If you sign in with a third-party provider, your use of that provider's service is governed by its own terms and privacy policy in addition to this Agreement.

You agree to provide accurate information about yourself and to keep it current, including the health information used to build your formula. If you believe your account has been accessed without your authorization, contact us at support@tailored-health.com. We may suspend or terminate an account that we reasonably believe has been used in violation of this Agreement. Our handling of the information associated with your account is described in our Privacy Policy.

Program name: TailoredHealth Alerts. The program is operated by Vitruvion.co LLC, doing business as TailoredHealth.

If you optionally provide a mobile number and check the separate, unchecked SMS consent box, you agree to receive automated customer care and transactional SMS from TailoredHealth. These texts are for account access (such as one-time login or verification codes) and customer support about your labs, formula, order, or account. We do not send marketing or promotional text messages. Consent is not a condition of purchase.

The phone field and SMS consent checkbox are shown during labs/account setup. A public description of that opt-in, including a screenshot of the Confirm details step, is at https://tailored-health.com/pages/sms-alerts.

Message frequency varies. Message and data rates may apply. Reply STOP to cancel. Text HELP for help. You can also contact customer care at support@tailored-health.com or through our contact page.

SMS opt-in data and consent will not be shared with third parties or affiliates for marketing or promotional purposes, except with vendors used solely to deliver messages. Additional details are in our Privacy Policy.

By providing your payment information at checkout, you authorize Tailored-Health.com (via our third-party payment processors, Shopify Payments and Stripe, depending on how you check out) to store your payment details and automatically charge that payment method for all applicable fees, including but not limited to one-time purchases, subscription renewals, taxes, and any additional charges related to your purchases or use of our services. You represent and warrant that you are authorized to use the payment method provided.

Your personalized formula is provided as a 90-day supply. Subscriptions automatically renew every 90 days and are billed once per 90-day cycle at the rate shown during checkout. You will be charged for the upcoming 90-day shipment at the start of each new cycle.

If you update your health assessment or profile, any adjustments to your formula will be reflected in your next 90-day shipment. Your subscription will continue to renew automatically until you choose to cancel or pause your plan.

You may pause or cancel your subscription at any time through your TailoredHealth dashboard or by contacting us at support@tailored-health.com.

To prevent being charged for your next 90-day renewal, cancellations must be submitted at least 14 days prior to your next refill date.

Once a billing cycle has begun, you are responsible for the full 90-day payment. Refunds are not issued for mid-cycle cancellations unless otherwise required by law. We reserve the right to rely on any notice of cancellation that appears legitimate and to cancel service on that basis. Our liability for improper cancellation is limited to reinstating your plan.

If your payment method fails or is declined, TailoredHealth may attempt to re-process the charge or request updated payment information. We reserve the right to suspend or terminate your subscription if timely payment cannot be completed. You are responsible for paying any applicable sales, use, or other taxes related to your purchase.

We reserve the right to modify pricing or billing practices at any time. Any such changes will be communicated to you prior to taking effect.

Because TailoredHealth formulas are custom-made and personalized to each user, all products are final sale and non-refundable once produced and shipped.

If you need to make a change to your order, please contact us within 24 hours of placing it at support@tailored-health.com.

After this period, orders are processed and cannot be modified or refunded. Refunds or replacements will only be issued in the case of verified shipping errors or product defects, at our sole discretion.

All website design, text, graphics, sound, software, and other content, and the selection and arrangement thereof, are the property of Tailored-Health.com or its licensors and are protected by United States and international copyright law. All rights to such materials are reserved to their respective copyright owners. You may not reproduce, distribute, or create derivative works from this content without our prior written permission.

Tailored-Health.com and its affiliates shall not under any circumstances be liable for any damages of any kind arising out of, in connection with, or relating to the use of or inability to use this site or the Services.

This Agreement shall be governed by and construed in accordance with the laws of the State of Colorado without regard to choice of law rules. Each user agrees to submit to personal jurisdiction in the State of Colorado and further agrees that any cause of action arising from or relating to the use of this site or this Agreement shall be brought exclusively in a court in the State of Colorado.

We may update, amend, or change this Agreement at our sole discretion and at any time. Updates will be posted on this page with a revised “Last updated” date. Your continued use of the site after changes are posted constitutes your acceptance of the revised Agreement. You are encouraged to revisit this page from time to time to review any changes that have been made.

This Agreement constitutes the entire and only agreement between Tailored-Health.com and each user of this site with respect to the subject matter of this Agreement and supersedes any and all prior or contemporaneous agreements, representations, warranties, and understandings, written or oral, with respect to the subject matter of this Agreement.
