This Terms of Use Agreement (“Agreement”) sets forth the agreement between Tailored-Health.com (“TailoredHealth,” “Tailored-Health.com,” “we,” “our,” or “us”) and each user (“you” or “user”) governing your use of this website. Please read this Agreement carefully before using this site. By accessing or using this site, you agree to be bound by this Terms of Use Agreement. If you do not agree to these terms, you may not access or otherwise use this site.
Health Information Disclaimer
The information provided on this site is for informational purposes only and is not intended as a substitute for advice from your physician or other healthcare professional, nor for any information contained on or in any product label or packaging. You should not use the information on this site for diagnosis or treatment of any health condition or for prescription of any medication or treatment. Always consult your physician or qualified healthcare professional before taking any medication or nutritional, herbal, or homeopathic supplement, before starting any diet or exercise program, or before adopting any treatment for a health condition.
Payment Plans & Authorization
By providing your payment information at checkout, you authorize Tailored-Health.com (via our third-party e-commerce platform, Shopify Payments) to store your payment details and automatically charge that payment method for all applicable fees, including but not limited to one-time purchases, subscription renewals, taxes, and any additional charges related to your purchases or use of our services. You represent and warrant that you are authorized to use the payment method provided.
90-Day Subscription Model
Your personalized formula is provided as a 90-day supply. Subscriptions automatically renew every 90 days and are billed once per 90-day cycle at the rate shown during checkout. You will be charged for the upcoming 90-day shipment at the start of each new cycle.
If you update your health assessment or profile, any adjustments to your formula will be reflected in your next 90-day shipment. Your subscription will continue to renew automatically until you choose to cancel or pause your plan.
Cancellations and Pauses
You may pause or cancel your subscription at any time through your TailoredHealth dashboard or by contacting us at support@tailored-health.com
To prevent being charged for your next 90-day renewal, cancellations must be submitted at least 14 days prior to your next refill date.
Once a billing cycle has begun, you are responsible for the full 90-day payment. Refunds are not issued for mid-cycle cancellations unless otherwise required by law. We reserve the right to rely on any notice of cancellation that appears legitimate and to cancel service on that basis. Our liability for improper cancellation is limited to reinstating your plan.
Payment Issues
If your payment method fails or is declined, TailoredHealth may attempt to re-process the charge or request updated payment information. We reserve the right to suspend or terminate your subscription if timely payment cannot be completed. You are responsible for paying any applicable sales, use, or other taxes related to your purchase.
We reserve the right to modify pricing or billing practices at any time. Any such changes will be communicated to you prior to taking effect.
Returns and Refunds
Because TailoredHealth formulas are custom-made and personalized to each user, all products are final sale and non-refundable once produced and shipped.
If you need to make a change to your order, please contact us within 24 hours of placing it at support@tailored-health.com
After this period, orders are processed and cannot be modified or refunded. Refunds or replacements will only be issued in the case of verified shipping errors or product defects, at our sole discretion.
