TailoredHealth is operated by Vitruvion.co LLC. TailoredHealth is a brand and DBA of Vitruvion.co LLC. The public website at tailored-health.com and the account portal at my.tailored-health.com are operated by Vitruvion.co LLC.

When completing your questionnaires, we ask general questions about your lifestyle, family history, medical history, and current medications. This information is used to develop a personalized supplement program recommendation for you. The information is provided for informational purposes only and is not intended as a substitute for any information contained on or in any product label or packaging. You should not use the information from any Tailored-Health.com for diagnosing or treating any health problem or disease, or prescribing any medication or other treatment.

You acknowledge that TailoredHealth may collect and process “personal information” (i.e., information that could be used to contact you, such as full name, e-mail address, phone number, and shipping address) in connection with the Service. Such information collected by TailoredHealth may be stored and processed in the United States, Canada, or any other country in which TailoredHealth or its agents maintain facilities. By using the Service, you consent to any such transfer of information outside of your country. TailoredHealth makes no representation or warranty with respect to any duty to permanently store any personal information you may provide.

Information you provide to us. Personal information you may provide to us through the Services includes:

• Contact data, such as your first and last name, email address, billing and mailing addresses, and phone number (including a phone number you optionally provide when starting with your labs or setting up your account).
• Profile data, such as the username and password that you may set to establish an online account on the Site, and any other information that you add to your account profile.
• Authentication data, such as your email address and, if you choose to sign in with a third-party provider, the basic profile information that provider shares with us.
• Social media data, such as the information provided in your bio.
• Health-related data, such as the information you provide to us when you fill out a survey, questionnaire, or other type of form through the website, including information about your health and wellness, health goals, diet, lifestyle, location information, demographic information, your views about health and wellness.
• Communications data, based on our exchanges with you, including when you contact us through the Site, social media, email, text message, or otherwise.
• Transactional data, such as information relating to or needed to complete your orders on or through the Site, including order numbers and transaction history.
• Marketing data, such as your preferences for receiving our marketing communications and details about your engagement with them.
• Promotion data, including information you share when you enter a competition, promotion, or complete a survey.

You can create an account and sign in either by verifying your email address with a one-time code or by using a third-party sign-in provider such as Google. If you choose to sign in with Google, Google confirms your identity to us and shares a limited set of basic profile information — your email address, name, and profile picture. We use that information only to create and sign you into your TailoredHealth account and to communicate with you about your formula and order. We do not receive your Google password, and we do not request access to your Gmail, Google Drive, contacts, calendar, or any other Google service. Your use of Google's sign-in service is also governed by Google's own privacy policy. You may stop using third-party sign-in at any time and continue accessing your account by email verification instead.

We use personal information to operate the Services and communicate with you, including to:

• Create and secure your account, verify your identity, and provide sign-in or one-time codes.
• Build, review, and deliver your personalized formula and related product information.
• Process orders, shipping, billing, and customer support.
• Send service-related communications about your account, labs, formula, or order by email and, if you provide a phone number, by text message.

Direct marketing, which may include using your personal information to:

• Send you direct marketing communications by email (you may opt-out of our marketing emails).
• Personalize our marketing communications according to your needs and interests.
• Monitor your interaction with our communications (such as whether you opened them).

We do not use phone numbers you provide through our labs get-started flow or account setup to send marketing text messages. Phone numbers are used only for secure account access and support or other service-related texts, as described below.

If you choose to give us a phone number — including when you optionally provide one in our labs get-started flow or when setting up your account — and you check the optional SMS consent box, we may use it to send automated customer care and transactional SMS from TailoredHealth (operated by Vitruvion.co LLC), including:

• One-time codes or other messages needed for secure account access and verification.
• Support and service-related updates about your labs, formula, order, or account (for example, when your formula is ready to review).

Providing a phone number is optional. The SMS consent checkbox is separate from your acceptance of these Terms and this Privacy Policy, and it is unchecked by default. We do not send marketing or promotional text messages. Message frequency varies. Message and data rates may apply. Reply STOP to end. Text HELP for help. You can also ask us to stop texts by contacting us at support@tailored-health.com or through our contact page.

SMS opt-in data and consent will not be shared with third parties or affiliates for marketing or promotional purposes. We may share your mobile number with vendors used solely to deliver messages (for example, our text-message provider) on our behalf. A public description of this opt-in, including a screenshot of the Confirm details step, is at https://tailored-health.com/pages/sms-alerts.

If you arrive at TailoredHealth through a paid advertisement, we may share limited information with the advertising platform so we can measure whether the ad led to account creation and improve our ads. We do this only for advertising measurement — not for selling your data.

• Meta (Facebook / Instagram). When you click a Meta ad and later verify your email to create an account, we may send Meta a conversion event that includes a one-way hash of your email address (SHA-256), together with Meta’s own click and browser identifiers when available. The hash cannot be reversed into your email by TailoredHealth, and we do not send Meta your lab results, questionnaire answers, health goals, or other health information with that event.
• Google Ads. When you click a Google ad and later verify your email, we may send Google an offline conversion that includes only Google’s click identifier and an event timestamp. We do not send Google your email address (hashed or otherwise), lab results, or other health information for that measurement.

These partners process the information under their own privacy policies. You can learn more in Meta’s Privacy Policy and Google’s Privacy Policy. Ad platforms may also offer controls for personalized advertising in your account settings with those companies.

TailoredHealth reserves the right, at its sole discretion, to update, amend and/or change this Privacy Policy without prior notice and at any time. Updates to this Privacy Policy will be posted here. You are encouraged to revisit this Privacy Policy from time to time in order to review any changes that have been made.
